A useful record should be short enough to read, specific enough to trust, and clear about what happens next.

1. Lead with observable impact

Start with who was affected, what changed for them, and when the condition began. Leave internal detail for the next section.

2. Keep the timeline factual

Use timestamps, short verbs, and named handoffs. Avoid guessing motives or compressing several actions into one vague sentence.

3. Separate correction from prevention

Mark the immediate fix, then list the changes that reduce repeat risk. Each follow-up needs an owner and a review date.
